Suzanne Elise Assisted Living

Suzanne Elise Assisted Living offers residents supportive services and assistance with activities of daily living. A professional needs assessment is done prior to move in to develop an individually tailored service plan and to determine the level of care needed. The service plan is reviewed 30 days after move in and every 90 days thereafter to assure that as a residents need change the service plan is adjusted accordingly.

Service Package

All service packages include:

Three full meals a day
Modified diets
Assistance with self medication management
Facility transportation
Coordination of medical, dental, vision appointments
Weekly housekeeping and laundry service
Group and individual activities
Cable television
Emergency call system

Additional supportive services available in tailored levels of care may include:

Medication administration or supervision
Grooming and dressing
Bathing
Mobility and transfers
Reminders or escorting to mealtimes and activities
Incontinent care
Cognition Assistance

Comprehensive Service Package

This is for those residents whose service needs, while at Suzanne Elise, have grown to include more personal care needs or who would otherwise benefit from additional support services including Hospice. More information on this service package is available upon request.
